Pennsylvania has required health screenings that must be done for homeschooling children. These are exactly the same as what your child was required to do while attending LCA.
-Immunization forms must be sent in with Affidavit.
Physical Exams
Physical exams are required for students going into the following grades:
1st, 6th, 11th
This form must be used for the physical exam.
Dental Exams
Dental exams are required for students going into the following grades:
1st, 3rd, 7th
This form must be used for the dental exam.
If your student requires one of these forms, turn it in with your affidavit, or by August 1. These forms go to your public school superintendent.
