You simply need to
Print off the following form (only one per family).
Fill it out.
Turn it in to the superintendent of your local public school district (their names and addresses are listed at the bottom of this page).
Get proof of receipt either by mailing and getting a signature with delivery, or dropping the form off yourself and getting a receipt from the receptionist.
You will need to keep track of each of your student's grades and curriculum throughout the school year. These should be compiled in a binder (known as a portfolio under PA homeschool law). An example is here. At the conclusion of each school year, each of your student's binders must be evaluation by a certified evaluator. The evaluation must be turned in to your school district's superintendent on or before June 30th each year. However, an arrangement should be made with a certified evaluator well in advance of this date. Evaluators can be found by county here. Each evaluator will look for different things and will want different pieces of material. Because of this, it is a good policy to contact your evaluator in advance so you can adjust your portfolio accordingly.
